Connect in Gippsland is a free program. It helps people with disability and their carers find the services and supports they need.
Who Do We Help?
Carers: People looking after someone of any age who lives with disability.
People with disability: People living with a disability, especially those between 16 and 25 years old.
What Do We Do?
Listen to your needs and understand your interests.
Find services and supports in your local area.
Work together with you to improve your wellbeing.
How Does it Work?
The program will match you with a staff member called a Connector.
Your Connector will help you explore your needs, interests and choices. These might include:
Making friends – meeting new people and joining social groups.
Peer support – talking to people who have similar experiences to you.
Finding services – getting in touch with organisations that can help you.
Your rights – helping you understand your rights and speak up for yourself.
School and work – finding pathways for you to learn new things or get a job.
New activities – trying out new hobbies and things you might like to do.
Your Connector will talk with you about what matters to you. Together, you will:
Talk about your choices, needs, and what you like to do.
Make a plan to help you feel happier, healthier and more supported.
Take the next steps to start using your plan.
